Insurance Forms Insurance Companies Use to Assess Your Injuries

If you have been injured in a motor vehicle accident, you likely received several forms from your insurance company. You may be unsure how to complete these insurance forms and how these forms will be used by your insurance company. There are strict deadlines for submitting the forms to your insurance company to qualify for Section B Accident Benefits. First Things First! Do Not Speak with the Insurance Company of the Other Driver

Before submitting forms to the insurance company or providing information by phone, be sure that the insurance forms you complete are from your own insurance company.

If you are a pedestrian or cyclist, you may be eligible for Section B Accident Benefits from the insurance company of the driver that hit you. Call us before speaking with the insurance company. AB-1: Notice of Loss and Proof of Claim Form

This is the initial insurance form you complete to notify your insurance company of your injuries in the car accident. On the AB-1 Form, you need to inform your insurance company of details such as the time and place of the accident and your injuries.

You are required to complete the AB-1 Form and return it to your insurance company within 10 business days of the accident in order to receive Section B Accident Benefits. If you are unable to submit the AB-1 Form to your insurance company within 10 business days of the accident due to your injuries, submit the AB-1 Form as soon as possible.

AB-1A: Claim for Disability Benefits Form

You will need to see a doctor to assess your injuries if you cannot work due to the accident. Submit this insurance form to your insurance company as soon as possible after the accident if you are unable to work. Your Accident Benefits Adjuster will require a certificate to be completed by your employer confirming your employment. If you are self-employed, you will be asked to provide information on your earnings. Parts 3 and 4 of the AB-1A form need to be completed by a physician, indicating the time in which you are unable to work.

AB-2: Treatment Plan Form

This form must be completed by your primary health care practitioner, such as your physiotherapist or chiropractor. The form outlines the treatment plan recommended to you by your primary health care practitioner and is sent by your primary care practitioner to your insurance company.

AB-3: Progress Report Form

This form is to be completed by your primary healthcare practitioner when requested by the insurance company. Your insurance company will periodically request an update from your physiotherapist or other treatment providers on your recovery and whether any additional treatment is required.

AB-4: Concluding Report Form

This form is to be completed by your primary health care practitioner and will provide a summary of your progress and whether any additional treatment is recommended.

AB-5: Referral to Injury Management Consultant Form

The AB-5 form is completed by your primary health care practitioner who is requesting consultation by an Injury Management Consultant. This form is typically completed by your primary health care practitioner to get another opinion on your injuries and treatment plan.
